[SPOILERS AHEAD!]

The Spartan 1 programm (Codename: Orion) went never public (as far I know) and the "MIA"-status was introduced with the first death of a Spartan II, not before.

For almost everyone, Johnson was a normal soldier, a though son of a bitch but not a spartan.
Very few knew about this specific part of his past and after 2506 all Orion-exclusive-personell operations ceased.

The first actual Spartan to be listed as KIA is Kurt-051 but I won't reveal the circumstances of his death, too much of a spoiler ;)

It's really nice, but the pedant in me can't help but point out that III's and IV's aren't supposed to be as big as the II's. Remember that the II's augmentation process also served to roughly standardize their proportions to ease manufacturing their armor and allow greater interchangeability of the parts, whereas as the III's are pretty close to normal human ranges in Reach and all over the place proportion-wise. Since the IV's are recruited from adults already serving in the UNSC armed forces I doubt they're grown any more than they already are.

According to the Essential Visual Guide, Noble Six is 6'9" out of armor, and (in the Halo 4 Essential Visual Guide) Thorne is 6'10" out of armor. They're both around the same height out of armor as most of the Spartan-IIs (John-117 being 6'10" himself).

Both groups had the same, if not better, augmentation processes that the Spartan-IIs had:
